د. يارا بشور؛ أغنيةٌ من الضوء على دروب الطب

- بدايةً أود أن أشكرك على تعاونك؛ إنّه شرفٌ لي أن ألتقي طبيبة عظيمة مثلك..

- شكرًا جزيلًا؛ أنا ممتنة لك ولِمنصتكم الكريمة على هذه الفرصة.

-هل من الممكن أن تُعطينا لمحة إلى سيرتك المهنية الطبية؟

- درستُ الطبُّ في جامعة أمستردام الحرة، وتخرَّجتُ عام 2013؛ ثُمَّ بدأت دراسة الدكتوراه في قسم الجراحة التجميلية والترميمية وجراحة اليد. وهناك؛ بدأت العمل على رسالتَيّ دكتوراه؛ إحداهما عن مضاعفات إعادة بناء الثدي باستخدام زرعات ثدي مصنوعة من السيليكون، والثانية عن مضاعفات مواد الحشو الوجهية (الفيلر filler)، ولأجل رسالتَي الدكتوراه هاتين كتبتُ 10 مقالات ونشرتها في مجلات أمريكية وأوروبية، وما زلت أعمل على 20 مقالة أخرى.

وبهدفِ إبلاغ جرَّاحِي التجميل عن اكتشافاتنا؛ قدمت عملي في العديد من البلدان؛ كهولندا وفرنسا وألمانيا وبلجيكا وإسبانيا وإيطاليا وغيرها ما يبلغ 28 دولةً في المجموع.

وفي عام 2015؛ كنت محظوظة بالفوز بالجائزة الثانية في برشلونة، وهذا العام فزت مرتين بالجائزة الثانية في روما.

ثم إنّه من الصعب جدًّا الحصولُ على تمويل في الجراحة التجميلية؛ لكنِّي كنت محظوظة في الحصول على تمويل من وزارة الصحة الهولندية لأجل البحث الخاص بمضاعفات مواد الحشو الوجهية، وفي الوقت الحالي، أُتِمُّ دراستي لدرجة الدكتوراه وأعمل مدرسةً لطلاب الطب في الجامعة الحرة،وعُدت إلى العيادات في المستشفى منذ بضعة أشهر في قسم الجراحة.

وفي عام 2012؛ مَثَّلت سوريا في اجتماع منظمة الصحة العالمية في لندن (وقد دعيت لإلقاء محاضرة في مؤتمر طبي في أمريكا العام الماضي؛ لكن للأسف لم أتمكن من الذهاب).

- هذا ملهم حقًّا، ومن المؤكد أنه استغرق كثيرًا من الجهد وساعات طويلة من الدراسة، ومن الواضح أنك مررت بكثيرٍ من المتاعب؛ لكنَّ إصرارك يعكس حبك مهنتَك؛ فكيف اخترتِ مهنتك؟ ولماذا اخترتِ هذا الاختصاص على وجه الخصوص؟

- اخترتُ أن أصبح طبيبة لقناعتي بأنَّ هذه المهنة تخفّف من آلام الناس، ولمساعدتهم في أن يصبحوا أصحاء مرّة أُخرى جسديًّا؛ فأخفف معاناتهم النفسية إثر إصابتهم بالتشوّهات الجسدية.

وفي أثناء فترة التدريب في قسم الجراحة التجميلية أدركت كم يعاني الناس نتيجة التشوهات الناتجة إمّا عن أذياتٍ وإمّا عن السرطان، وأردتُ أن أفعل شيئًا لإزالة التشوه وتحسين المظهر الجسدي؛لأن المظهر مهم للغاية وله تأثير نفسي كبير على الأداء الاجتماعي.

- هذا جميل! لقد ذكرتِ منذ قليل صعوبات التمويل التي واجهتك وقد استطعتِ التغلب عليها؛ لكن من المؤكّد أنَّها لم تكن الصعوبات الوحيدة التي مررتِ بها؛ فالحصول على قبول في دراسات الجراحة التجميلية ليس أمرًا سهلًا ويتطلب كثيرًا من الجهد؛ فهلا حدَّثتِنا عن الصعوبات التي واجهتك في مسيرتك؟

- كان هناك كثيرٌ من الصعوبات؛ بدايةً؛ إنَّ اللغة الهولندية لغة صعبة ولكي أطور نفسي كان عليَّ أن أتعلمها، ثمّ إنَّ الثقافةَ والعادات الهولندية مختلفة أيضًا عن الثقافة والعادات السورية، وقد كان عليّ أن أجد طريقةً للتأقلم مع هذه الثقافة مع المحافظة على جذوري السورية، وبالطبع كنت أحنُّ كثيرًا إلى بلدي سورية الجميلة وعائلتي، وكان موضوع الدراسة الذي اخترته صعبًا أيضًا؛ فهو متعلق تعلُّقًا وثيقًا بكلٍّ من علم المناعة وعلم الأحياء الدقيقة وعلم الوراثة، وهي موضوعاتٌ صعبةٌ للطبيب؛ فكنت أقضي ساعات طويلة في فهمها.

- أجل، أقدِّر شعورك هذا.. ولكن؛ ماذا عن كونك امرأةً أيضًا؟ هل كان لذلك أيُّ أثر على مهنتك؟

- إجراء البحوث عملٌ دقيق للغاية، والمرأة عمومًا دقيقة جدًّا، وقد يستغرق الأمر سنواتٍ لأداء البحث ويتطلب كثيرًا وكثيرًا من الصبر؛ لكن من ميزات المرأة أنها صبورة، وقد عملت مع أكثر من 100 شخص في أبحاثي؛ لذا كان من المهم أن أكون اجتماعيًّة للغاية، وأعتقد أن معظم  الميزات التي تمتلكها المرأة تؤهلها للوصول إلى مبتغاها.

- لقد أحببتُ ذلك، إنه محفّز حقًّا .. أحببت كونَ الصفاتِ الأنثوية مطلوبة من أجل إتمام الأبحاث، وهذه نقطة تستحق النشر!

- أعتقد أنَّه قد حان الوقت للنساء لأن ينهضنَ ويتبعنَ أحلامهنَّ.

- أجل، أنت محقّة.

عامةً؛ تخبرنا النساء العظيمات عن الضغط الذي يسبّبه كونهنّ نساء فيما يخصّ المجال المهني؛ ولكني أحببتُ أنَّ كونك امرأة كان له أثر إيجابي في مسيرتك.

- لكن الأمر يتعلق بطريقة النظر إلى الموضوع؛ فيجب التطلُّع إلى جوانبكِ الإيجابية؛ إذ تمتلك النساء العديد من الأدوات الرائعة وهذه هي قُواهنّ الخارقة.

- أجل، إنه أمر يتعلق بالعقلية.

- بالضبط!

- وفي حين أنك تمتلكين هذه النظرة المستقبلية العظيمة وهذه العقلية المذهلة؛ إلى أي أساسٍ استندتِ في اختيار موضوعات أبحاثك؟ وكيف تتخيلين إمكانيةَ أن تسهمَ أبحاثُك في تطور البلد؟

- تتمحورُ أبحاثي عن مضاعفات استخدام زرعات الثدي المصنوعة من السيليكون وعن مضاعفات مواد الحشو الوجهية؛ فأنا أتحقق من الأسباب التي تجعل المريض يعاني تلك المضاعفات؛ مثل الإنتان والالتهاب وَرد فعل الجسد تجاه الجسم الأجنبي والنخر.

ونحن نبحث في دور البكتيريا ودور جهاز المناعة وأي استعداد وراثي، وأنا أعمل على هذا الموضوع مع مجموعة من إسبانيا والعديد من المختبرات في هولندا؛ متضمّنةً مختبرًا تابعًا لوزارة الصحة.

وإجراءُ البحوث يساعد في تطور البلد، ودون إجراء البحوث لا يمكن الوصول إلى الأفكار الجديدة؛ فالأفكار الجديدة ضرورية لتطوير معرفتنا وصنع القرار في المجال الطبي ثُمَّ مساعدة شعبنا للتطور بأفضل طريقة ممكنة.

- هذا رائع..

ففي عالم مليء بالعمليات التجميلية الضرورية في العديد من الحالات ثمّ إنَّ معدلات الأخماج والالتهابات هي أكثر عرضة للازدياد وتحتاج إلى معالجتها جيدًا. وبذلك كلّه ما أهدافك المستقبلية؟

- أنا أطمحُ أن أصبح جراحة تجميل ناجحة؛ ولكنني أستمتع أيضًا بالأبحاث ومشاركة المعرفة عن طريق التعليم، وبالطبع فإنَّ حلمي في أحد الأيام أن أستخدم خبرتي لخدمة شعبي وبلدي الجميل، وأودُّ أن أساعد الآخرين وأشجعهم على تحقيق أحلامهم.

-أحببت طريقتك في التفكير، إنها محفّزة ومليئة بالطاقة الإيجابية.

هذا الصدق في تعاملك مع الإنسانية عامةً وبلدك خاصةً يعكس جانبك الاجتماعي والمبادر، وهذا بالطبع من أهم الصفات المطلوبة للعمل التطوعي..

فما رأيك بالعمل التطوعي؟ وما آثاره في الفرد والمجتمع؟

- عندما كنتُ طالبة طب تطوَّعتُ للعمل في الصليب الأحمر، وكانت واحدةً من أكثر التجارب المدهشة؛ فقد حصلتُ على فرصة تعلُّمِ مساعدة الآخرين دون توقع أيّ شيء مقابل ذلك؛ لكنني شعرت بالمعنى السامي للمهنة أيضًا دون التفكير في الماديات.

شيء سامٍ أن يساعد الناس بعضهم بعضًا دون التفكير بالمقابل، وأعتقد أنه بهذه الطريقة يساعد المجتمع بعضه بعضًا وهكذا يكبر وينهض.

- وطبعًا أن تكوني جزءًا من منظمة مهمة كالصليب الأحمر؛ فهذا يترك أثرًا في حياة الأشخاص الذين يعانون من مختلف أنواع الصعوبات، وربَّما دعمُ المرأة بمختلف الطرائق يُعدُّ أحد الموضوعات العاليةِ القيمة التي تركّز عليها المنظمات.

برأيك، كيف يمكننا تمكين المرأة في المجال الطبي؟

- يجب أن ندعم المرأة في الحصول على مزيدٍ من الثقة بالذات، ويجب أن نعطي الفرصة للمرأة ليَثبُتَ لها أن لديها القدرات الرائعة للوصول إلى الهدف وتطوير نفسها؛ فعلينا أن نتخلص من الصورة النمطية لعمل المرأة؛ إذ يمكن للمرأة أن تفعلَ ما تشاء فإذا أرادت أن تصبح طيارًا فينبغي لها أن تصبح طيارًا ببساطة.

والمرأة لديها القدرة على فهم معاناة المريض وخوفه حتى لو لم يتحدث عن ذلك، ويمكن للمرأة أن تشعر إذا كان هناك شيء مخفي كالقلق مثلًا ثُم تُزوِّد مريضها بالطمأنينة التي يحتاجها.

- أنا حقًّا مذهولة! وهذا يحفّزني لإتمام مسيرتي الطبية، ولأنّني طالبة طب أستطيعُ أن أُخمّن أن طلاب الطب سيفرحون بكلمة أخيرة منك؛ فماذا تقولين لهم جميعًا؟

- في البداية؛ على طالب الطب أن يدركَ أنَّ الثقة بالذات هي حجر الأساس الذي يبني عليه الطبيب مستقبله، ثمَّ إنَّ التصميمَ والمثابرة للوصول إلى الهدف هي أهم الأشياء لتحقيق الحلم؛ فبناء منزل يستغرقُ كثيرًا من الصبر والعمل، لكنّك في النهاية سترى النتيجة المطلوبة.

كذلك لا تستسلم فسوف تسقط عدة مرات وهذا جيد ما دمتَ لا تنسى أن تنهض وتذهب إلى الهدف!

وفي نهاية هذا اللقاء؛ أريد أن أشكركم على هذه الفرصة العظيمة لإيصال قصتي إلى عموم الناس.

M: first of all i really want to thank you for your collaboration, and i am so honored to have the chance of making an interview with such a great doctor.

Y: Thank you very much. I am grateful for you and your platform for this great opportunity.

M: Can you please tell us about your medical status?

Y: Sure.

I studied medicine at the Free University of Amsterdam and graduated in 2013.

Then I started as a PhD student at the department of plastic, reconstructive and hand surgery

There, I started to work on 2 PhD's; one about complications of breast reconstruction with silicone breast implants and one about complications of facial fillers.

For these PhDs I have written and published 10 papers in American and European journals and I am still working on 20 others

To inform plastic surgeons about our finds, I have presented my work in several countries

In the Netherlands, France, Germany, Belgium, Spain and Italy. 28 in total.

In 2015 i was luck to win the 2nd price in Barcelona and this year two times the 2nd price in rome

It is very difficult to get funding in plastic surgery, but I was lucky to win funding from the Dutch Ministry of Health on complications of facial fillers

At the moment I am finishing my PhD and working as a medical teacher at the Free University

I went back to clinics a few months ago at the surgery department

In 2012 I have represented Syria in A WHO (world health organisation) meeting in London. And I was also invited to present my work in America last year, but unfortunately I couldn't go.

I guess this is it for now.

M: that's so inspiring

and for sure that took a lot of work and long hours of study..

i can tell you've been through a lot,but your determination reflects your love for your career.. so how did you choose your career and why did you choose such a specialization?

Y:I chose to become a doctor to enlighten the pain of people. To help them to become healthy again physically and therefore also mentally.

During an internship at the plastics department I realised how much people suffer from any deformity after an injury or cancer. I wanted to do something about the deformity and physical appearance. Because the appearance is very important and has a huge psychological impact on the social functioning.

M: so lovely!

you've mentioned before the funding difficulties you've managed to get through.. but this for sure is not the only one you've faced because as it is known, getting applied for plastic surgery studies is very difficult and requires a lot of work ..so can you tell us about the difficulties you faced during your journey?

Y: There were a lot of difficulties.

First of all, the language. Dutch is a difficult language, but in order to grow I had to learn it.

Also, the Dutch culture is complete opposite of the Syrian culture. I had to find a way to get along with this culture, but also stay close to my Syrian roots.Of course, I also missed my beautiful country and family. The subject that I’m working on was also difficult, it is a lot about immunology, microbiology and genetics. There are difficult subjects for a doctor, I spend lots of hours to understand it.

M: yes i can feel you.

and what about being a woman? did this affect any side of your career?

Y: Doing research is very precise work. Women are generally very precise.

It takes years to perform research and it takes lots and lots of patience, female patience. Also, I worked with more than 100 people on my research, so it was important to be very social. I think there are all characteristics than women own and could help them a lot.

M: I loved that! this is so motivating. I loved that female characteristics are required to get through researches.. this is a point that worths spreading!

Y: I think it is now the time for women to get up and pursue their dreams.

M: you're right! I couldn't agree more.

In general, great females tell us that being a female causes a lot of pressure in the professional field ..

I liked the way it had a positive impact on your journey.

Y:  But it is how you look at it. You have to use your positive sides . Women have some amazing tools, these are their superpowers.

M: yeah.. it's all about the mentality.

Y: Exactly!

M: And while you have this great future view and this amazing mentality, how did you choose the subjects of your researches? how did you imagine that your researches would contribute in the development of a country?

Y: My research is about complications of breast reconstruction with silicone breast implants and about complications of facial fillers. I am investigating why patient develop complications such as infection, inflammation, foreign body reaction and necrosis. We investigate the role of the  of bacteria, the role of the immune system and any genetic predisposition. I am working on this manner with a group from Spain and several laboratories in the Netherlands including a laboratory from the Ministry of Health. Research is important for new insights, these insights are needed to develop our knowledge and decision making in the medical field and therefore to help our people the best possible way.

M: wow this is so great..

in a world where plastic surgeries became a must in several cases, infections and inflammations rates is more likely to rise and should be treated very well..

And what are your future goals and plans?

Y:I would like to become a successful surgeon. But I also enjoy doing research and sharing the knowledge through education. Of course, it is my dream, one day, to give something back to my people and beautiful country. I would like to help and encourage others to pursue their dreams.

M:i like the way you think.. it's so motivating and full of positive energy.. Being so genuine for humanity in general and especially for your country reflects your social and initiative side.. And this is for sure the first  characteristic required for volunteering..

what do you think about volunteering and what are its effects on the individual and the society?

Y: When I was a medical student I have volunteered for the Red Cross. It was one of the most amazing experiences. I got the chance to learn to help others without expecting anything back from it, but yet I felt the most valuable. I think this is how a society helps each other and how a society grows.

M: And of course being a part of such great organisations can leave an impact on people's lives dealing with different kinds of difficulties, and maybe one of the high valued topics organisations try to focus on is supporting women with several possible ways..

In your opinion, how can we support women in the medical field?

Y:We should support women in gaining more self-esteem. They have to realise that they have some amazing capacities to reach their goals and go for it. But we should also give them the opportunity to express themselves and develop themselves. We have to get rid of the stereotype women work. A woman can do whatever she wants. If she wants to become a pilot, she should become a pilot. Women have a natural feeling for what the patients tells when he is not speaking. They can feel if there is something hidden for example any anxiety. And they can help to comfort the patient when needed.

M: I so impressed. This gives me a lot of motivation for my medical journey.. And as a medical female student i know that medical students would be so happy to receive one more word from you.. what would you tell them all out there?

Y:First of all, you got to have faith in yourself. And it takes perseverance to reach your dream. It takes lots of work to build a house, stone by stone. But in the end  you will see the result.

Also, don’t give up. You will fall many times. That is oké, as long as you don’t forget to get up and go for it!

M:  this is so inspiring, so motivating and so encouraging !
